CORTISOL STIMULATION TEST

Test Documentation
Specimen Required
Draw SST for baseline cortisol before medication is administered. Centrifuge, separate and send 1.0 mL serum (0.5 mL minimum) refrigerated in a screw-capped plastic vial. Label with COR0MIN label. After ACTH (Cosyntropin) injection draw SST at 30 minutes and again at 60 minutes. Process samples the same as baseline sample. Label 30 minute serum with COR30MIN, and 60 minute serum with COR60MIN label. Actual collection times must be on each tube.

Alternate Specimen
Serum: Red-top

Rejection Criteria
Hemolyzed or lipeimc specimens, plasma

Methodology
Chemiluminescence

Stability
Room Temperature: 8 hours; Refrigerated: 72 hours; Frozen: 14 days

Reference Range
For a standard dose of cosyntropin administered after collection of baseline Cortisol, a rise in Cortisol greater than 18 ug/dL within 60 minutes demonstrates a normal result.
A baseline ACTH sample is also recommended. An elevated baseline ACTH level suggests primary adrenal failure.
