Specimen Required
Collect specimen 8-12 hours after oral dose. Measure at least two weeks after initiating treatment. Draw blood in a plain red-top tube. Centrifuge, separate serum from cells within 6 hours and send 2.0 mL seurm (1.0 mL minimum) refrigerated in a screw-capped plastic tube.
Submit with order: Perphenazine dose, Perphenazine dose frequency, Perphenazine route, Perphenazine type of draw.

Alternate Specimen
Plasma: EDTA, K2EDTA, sodium or lithium heparin
